Service Description
All parties booked MUST pay a deposit of $150.00. Your deposit is only to secure the date and time of your event. R DATE OR TIME MAY NOT BE CHANGED within five (5) days prior to the date of the party. Shooting Stars WILL NOT REFUND OR EXCHANGE YOUR DEPOSIT/PAYMENT after that date. All requests must be made in writing. Only 1 party may be booked at one time. Gold Star Package One: (1) court use (3 hours) Up to 20 kids. One (1) Field Attendant. Exclusive Party Room Includes: Two (2) Large Pizzas & two (2) 2 Liter Sodas, Birthday Banner, Soccer table cloth, 20 plates, 20 cups, Napkins & Utensils. Add the second field for only $80.00 per hour (Includes 2nd Coach. subject to availability) must advise at least 48 hours in advance. Additional silverware/plates, etc. will not be provided, all additional silverware will need to be provided by the host. The second field will be closed, unless rented, no exceptions.
Cancellation Policy
In consideration of participating in the sport of soccer, and for other good and valuable consideration, I hereby agree to release and discharge from liability Shooting Stars Indoor Soccer Center/Facility and its owners, directors, officers employees, agents, volunteers, and all other persons or entities acting for them (hereinafter collectively referred to as “Releasees”), on behalf of myself and my children, parents, heirs, assigns, personal representative and estate, and also agree as follows: 1. I acknowledge that the sport of soccer involves known and unanticipated risks which could result in physical or emotional injury, paralysis or permanent disability, death, and property damage. Risks include, but are not limited to, broken bones, torn ligaments, bruises, and other bodily injuries caused by contact with soccer balls, other participants, structures like goals, or spectators or other obstacles on the sidelines, or caused by uneven ground; medical conditions resulting from physical activity; and damaged clothing or other property. I understand such risks simply cannot be eliminated, despite the use of safety equipment, without jeopardizing the essential qualities of the activity. 2.I represent that I have adequate insurance to cover any injury or damage I may suffer or cause while participating in this activity, or else I agree to bear the costs of such injury or damage myself. I further represent that I have no medical or physical condition which could interfere with my safety in this activity, or else I am willing to assume – and bear the costs of – all risks that may be created, directly or indirectly, by any such condition. In the event that I file a lawsuit, I agree to do so solely in the state where Releases’ facility is located, and I further agree that the substantive law of that state shall apply. 3. I hereby give permission for images of my child or myself , captured during play in above named facility through video, photo and digital camera, to be used solely for the purposes of Shooting Stars Indoor Soccer Center/Facility promotional material and publications, and waive any rights of compensation and ownership thereto. ALL payments must be made prior to the day of training All players MUST arrive on time, a grace period of 10 minutes will be allowed, make up time will not be provided You must cancel 24 hours in advance, if you do not you will get charged your full session.
